QualityCare ACO brings strategy, operations and technology together to support the physicians caring for older adults, homebound patients and people living in assisted living or skilled nursing. We work as an extension of each practice, connecting facility staff, families and care teams while respecting physician independence.
QualityCare ACO participates in the CMS LEAD Model beginning with Performance Year 2027. ACO participant representatives hold 75 percent of board voting power, keeping the physicians and practices delivering care at the center of our direction.
